New patients must be able to prove they have been diagnosed and are currently symptomatic by producing medical records from the last year. Records must be related directly to the diagnosis you are qualifying under. We understand collecting medical records can be time consuming and frustrating, please stop by our office and we can obtain your records for you at no cost to you.

  • Severe Anorexia/Cachexia

  • Severe Chronic Pain

  • Spinal Cord Damage with Intractable Spasticity

  • Ulcerative Colitis
